The continuing care nursing team provide support to children and young people with complex needs through assessment to help identify any nursing needs as part of a specific care package of support. Assessments are made by senior nursing staff and recommendations made to Southwark’s MASH panel. MASH is Southwark Multi Agency Safeguarding Hub team for children. It brings together professionals from health, social care and education to look at and make decisions regarding the level of support required for children with complex needs. The team provide ongoing review of the nursing input within continue care packages and work closely with the community children nursing team and children with disabilities social work team.
