To provide nursing support and advice for children and young people with special needs and/or complex medical needs within Southwark. Working alongside school nurses and schools the team provides specialist nursing advice, training and continence assessment to support the inclusion of children and young people in an educational setting. The team also support the family in meeting the child or young persons needs at home.

Referrals are accepted from GPs, health visitors, school nurses, schools, other professionals and voluntary sectors using the multi-agency specialist service referral form. Parents and carers can also self refer by telephone. |
